POLICIES AND AGREEMENTS

 

Liability Disclaimer
I understand that dancing and dance classes consttute social/athletc/physical actvites and that injuries may occur. I am willing and able to accept full responsibility for my child's safety and well-being. Acknowledging this, I hereby release and agree to hold harmless Premier Dance School and its employees from any and all claims and liabilites, which may arise out of my child's partcipaton at the Premier Dance School.

Cancellaton/Withdrawal
You can cancel or pause your enrollment at any point in the season, and we do not charge any withdrawal fees. Please just provide us with written notice by e-mail before the start of a new session. Bear in mind, once you begin a session, tuition paid for that session is not refundable unless the withdrawal is due to serious, documented illness or injury.

Missed classes
No transfers, credits or cash refunds will be given for missed classes. Students who missed classes are still obligated to pay the full tuition. However, classes missed due to illness may be made up within a two-week period of the absence. If we have alternative classes, students may schedule a make-up class with their teachers. In case of documented, prolonged illness or injury (lasting more than a month), tuition may be put on hold until the student’s return.

Insufficient class enrollment class enrollment
The school reserves the right to cancel a scheduled class before the start of a new session because of insufficient enrollment.

TUITION PAYMENT CREDIT/DEBIT CARD AUTHORIZATION FORM

 

Tuition payments will be taken monthly September through May. Tuition will be charged to your card through auto-pay on the first day of your enrollment, and subsequently on the first day of each session.

I hereby authorize Premier Dance School to charge my credit/debit card for one time registration fee of $ 25.00 and monthly tuition of $
(please see our tuition table ). If classes are added or dropped after registration, the tuition amount will be adjusted accordingly. I understand that the debit/credit card transaction will occur monthly on the first day of each session.
